Anesthesiology&Operating Theater
In the past decade, Department of Anesthesiology and Operating Theater has made great progress in staff team building, health care, medical education and scientific research. It has enhanced the academic reputation at home and abroad, and has been nominated twice for “Best Anesthesia Department Ranking in China”, which was published by the Hospital Management Institute, Fudan University. The department is training base for doctor of medical, postdoctor and master of medical of anesthesia speciality, as well as national demonstration base of standardized training of residents, national base for pharmaceutical clinical trial, northern center of Chinese society of Anesthesiology (CSA)’s large database for geriatric anesthesia. It is also the institute of standing committee member and secretary-general of CSA, institute of secretary-general of Operating Room Nursing Committee of Chinese Nursing Association, and institute of group leader of geriatric anesthesia of CSA. In 2017, anesthesia department led the establish of National Alliance of Geriatric Aesthesia(NAGA), which includes 165 Class A/B tertiary hospitals nationwide.
Department of Anesthesiology and Operating Theater has 187 anesthesia doctors and nurses, of which 30% have doctor degree and 97% have master degree, with total 14 staffs have acquired senior titles. The clinical working area includes 24 central operating rooms that containing 2 hybrid operating rooms and 1 intraoperative radiation therapy operating room, 4 interventional diagnosis and treatment operating rooms, 3 functional neurosurgery operating rooms, 1 painless gastrointestinal endoscope operating room, 1 labor analgesia operating room, 8 postanesthesia care unit (PACU) beds. Anesthesia preoperative evaluation clinic started at 2017. 12 more operating rooms containing 1 MRI operating room, 1 hybrid operating room and 2 interventional diagnosis and treatment operating rooms will be put into use as The China International Neuroscience Institute(China-INI) is enabled at the end of 2018. Total 42 operating rooms will be available by then.
Nearly 20,000 cases of anesthesia were performed in Xuanwu Hospital on yearly average, of which 19% are critical patients and 40% are elderly patients. No death due to anesthesia occurred in 5 years. Applying the concept of Enhanced Recovery After Surgery(ERAS), our department has improved the anesthesia management strategy in the last 3 years, by which we promote postoperative recovery, shorten postoperative hospitalization time, and reduce medical expenses. The department, leaded by Prof. Wang Tianlong, places the highest priority on quality and safety of medical care. By optimizing regulatory framework, setting up reporting system of critical patients, checking handover sheet of patient under anesthesia, making routine operation of neurosurgery and pediatric epilepsy surgery, introducing guiding opinion of postoperative analgesia, and leading the formulate of a series of expert consensus and guiding opinion including “Expert Consensus on Perioperative Anesthesia Management of Chinese Geriatric Patient” as the institute of group leader of geriatric anesthesia of CSA, we ensure to deliver the highest quality and safest care possible. Through the unified management of antibiotics, anesthetics and high-value consumables, the effectiveness and safety of operating room are improved. Our department takes neurosurgical anesthesia and geriatric anesthesia as key disciplines. To provide high-quality care for patients with fragile brain function and critical condition, we are one of the earliest to use non-invasive cerebral oximeter, minimally invasive continuous cardiac output monitor, BIS monitor, functional continuous non-invasive arterial pressure hemodynamic monitor, portable ultrasonic apparatus and transesophageal ultrasonic monitor, transcutaneous carbon dioxide/oxygen monitor, muscle relaxation monitor, closed-loop target controlled infusion system, and blood cell salvage. Total 26 new medical technique and technology were carried out in the past decade, by which we promote the development of anesthesiology.
Aiming at being an academic and learning department, we have continuously improved continuing medical education and resident physician training. By giving daily morning class at 7:30, including “knowledge update” on Monday, “case discussion” on Tuesday, “resident’s round” on Wednesday, “resident’s basic training” on Thursday, “new technology/teaching round” on Friday, we promote sustainable personnel training, medical safety and quality, and resident physician training. Each year, we undertake teaching assignment of five-year and seven-year medical students, graduate students and students of international colleges of Capital Medical University.
Department of Anesthesiology and Operating Theater was the first "Anesthesia Simulation Training Base" awarded by the CSA in 2009. A national continuing education program “Simulation Training Class for Anesthesia Management in Critical Patients” was held for 6 consecutive years, over 100 simulation training instructors were trained. Another national continuing education program “Update in Clinical Technology of Geriatric Anesthesia”, which was approved in 2014 due to the department’s leading position in geriatric anesthesia, has been held for 5 times by now. In the assessment of International Medical Education Accreditation for Capital Medical University in 2014, the HPS simulation teaching of our department had gained the recognition of evaluation experts and had won the honor for the Capital Medical University.
As a research-focused department, more than 10 anesthesia medical doctor were send to overseas top medical centers for post-doctoral training to improve their research capacity in anesthesiology and perioperative medicine. We now have won 5 National Natural Science Foundation of China programs and over 20 provincial and ministerial-level programs, with total fund over 12 million CYN. Our main research fields are: 1) basic and clinical research related to geriatric anesthesia; 2) basic and clinical research on perioperative functional protection of fragile organ; 3) building anesthesia simulation teaching software and anesthesiology assisted teaching system. To engage international academic exchanges, Prof. Daqing Ma form Imperial College London is invited as overseas talent of Xuanwu Hospital, Prof. Lingzhong Meng from Yale University and Niccolo Terrando MD from Duke University are invited as guest professors. 45 SCI papers were published so far and over 240 papers were published on national core journals. Prof. Tianlong Wang, the academic leader, is a well-known professor and doctoral tutor, and takes position in several academic organizations and editorial board of core journals. He pays attention to the career development of young doctors. Person-times of staff serving as vice chairman, secretary general, vice group leader and member in CSA and provincial and municipal society of anesthesiology is 19. In 2014, An open source journal “Journal of Anesthesia and Perioperative Medicine, JAPM” was launched by Prof. Tianlong Wang and Prof. Jin Liu from West China Center of Medical Sciences, Sichuan University, and they two both act as co-editor-in-chief.
Personnel training of nurses is a critical part in the construction and development of the department. Chief nurse Ting Liu along with all the nurses have made tremendous efforts in improving nursing quality by electing “chief nurses” for sub-specialty of surgery, sending nurses overseas for neurosurgery nursing training, and so on. Her outstanding management and academic achievement have gained the recognition of her peers. She has been a member of Chinese Nursing Association since 2013, and was elected as secretary general of Operating Room Nursing Committee in 2017.
Leadership group concerns about the wellbeing of staffs, and the social benefit as well. To improve working environment, we have purchased water dispenser and coffeemaker, started yoga course, and set up ping-pong table. We have sent our best staffs to assist the earthquake relief in Wenchuan, to participate in anesthesia aid program in Qinghai and Inner Mongolia, to join the medical team of aid to Guinea and Trinidad and Tobago, and to assist and guide the work in 8 Class B and C secondary hospital in Beijing.
In the spirit of “benevolence”, Department of Anesthesiology and Operating Theater will keep on its culture and team building, and make contributions to Chinese health care.
